I authorize Veterinary Eye Clinic Monterey Bay to diagnose, prescribe, and treat my pet at the discretion of the veterinary team. Medical records may be obtained from my pet’s primary veterinarian and/or other veterinary offices my pet has visited. I allow the use of my pet's medical information including images, records and archived samples to be shared for analysis in retrospective studies that will help further the understanding of similar ophthalmic conditions. I understand that payment is due in full when services are rendered, no later than at the time the patient is discharged. To avoid a cancellation or no-show fee of $100, at least 24 hours’ notice is required to cancel or reschedule an appointment.
